Repatha Alternatives: Six Options Compared
The closest Repatha alternative is Praluent, which works the same way and has its own outcomes trial. Leqvio, Lipfendra and Lerochol are the other PCSK9 options, and ezetimibe and bempedoic acid fit when you need a smaller LDL drop.

Average result stays above 70. A second medicine can close the gap.
- Ezetimibeabout 98 to 104 mg/dL
Average result stays above 70. A second medicine can close the gap.

Average result stays above 70. A second medicine can close the gap.
The dark line marks your goal. Starting LDL: 130 mg/dL.
Rough estimate. Trials report the average drop for one medicine, usually compared with placebo and often in people already on a statin, and your result can differ. Use the LDL you have now, on your current treatment. Your clinician sets your goal. Figures match the guides on this site; see each medicine’s guide for the sources.
- Lerochol$2,388 a year
$199 a month direct from LIB Therapeutics
- Praluent$2,700 a year
$225 a month self-pay through GoodRx
- Repatha$2,868 a year
$239 a month cash through AmgenNow
- Lipfendra$3,780 a year
About $315 a month list price
- Leqvioabout $10,760 a year
$3,587.73 a dose list price for uninsured patients; most people use insurance
As of October 2026. Cash and list prices come from the manufacturers’ programs and announcements linked in each guide. Most insured patients pay less, and Leqvio is usually covered through insurance. Daily generic pills such as ezetimibe cost far less and are left out. Confirm current prices before you fill.
What are the alternatives to Repatha?
The main ones are Praluent, Leqvio, Lipfendra, Lerochol, bempedoic acid and ezetimibe. Repatha lowers LDL by about 60% and has outcomes trials, so people usually look elsewhere for cost or coverage, needles, side effects, or because they need less LDL lowering.
What drug is most similar to Repatha?
Praluent (alirocumab). It is the same kind of antibody that blocks PCSK9, with similar LDL lowering. You self-inject it every 2 weeks, or a higher dose every 4 weeks.
In ODYSSEY OUTCOMES, nearly 19,000 people who had recently had a heart attack or unstable angina took Praluent or placebo on top of a statin. Praluent lowered the risk of major cardiovascular events by 15%. Insurers often cover one of the two and not the other.
Is there a pill or a shot with fewer injections?
Leqvio (inclisiran) is an siRNA that tells the liver to make less PCSK9. A healthcare professional gives it once, again at 3 months, then every 6 months. It lowered LDL by about 50% in its main trials, and its outcomes trials have not reported as of October 2026.
Lipfendra (enlicitide), approved in July 2026, is the first oral PCSK9 inhibitor. It lowered LDL by 56% and 59% versus placebo at 24 weeks in the CORALreef trials. It launched at a list price of $315 a month, and its outcomes trial is underway.
Lerochol (lerodalcibep) is a once-monthly self-injection that can stay at room temperature for up to three months. It lowered LDL by about 56% versus placebo over a year, has a direct cash price of $199 a month, and has no outcomes results yet.
How does bempedoic acid compare with Repatha?
Bempedoic acid (Nexletol) lowers LDL by about 20%, versus about 60% for Repatha, so it fits best when you are relatively close to your goal. In CLEAR Outcomes, about 14,000 people who could not take statins had about 21% lower LDL than placebo and 13% fewer major cardiovascular events.
The 2026 ACC/AHA multisociety guideline elevated bempedoic acid to a first-line non-statin option.
Is there a cheaper alternative to Repatha?
Ezetimibe is usually the least expensive option. It is a generic daily pill that blocks cholesterol absorption in the gut and typically lowers LDL by roughly 20 to 25% when added to a statin. In IMPROVE-IT, adding it to a statin after a heart attack or unstable angina modestly lowered cardiovascular events.
Among PCSK9 options, Lerochol has a $199 a month direct cash price as of October 2026, and Repatha itself has Amgen's AmgenNow cash program.
Which Repatha alternatives are easier to tolerate?
No trial compares them head to head, so none can be called easier to tolerate. Label figures come from different trials, so use them as a list of things to ask about. Injection-site reactions ran 5.7% versus 5.0% on placebo for Repatha, 7.2% versus 5.1% for Praluent, 8% versus 2% for Leqvio, and 12% versus 5% for Lerochol.
The pills avoid injection-site reactions. Bempedoic acid's label lists higher uric acid, gout and tendon rupture (0.5% versus 0% in the cholesterol trials), so ask your clinician first if you have tendon problems.

Common questions
▸Which is better, Praluent or Repatha?
They are very similar. Both are antibodies that block PCSK9, both lower LDL by roughly half or more, and both have outcomes trials showing fewer cardiovascular events. The choice often comes down to which one your plan prefers.
▸Is there a pill that works like Repatha?
Yes. Lipfendra (enlicitide), approved in July 2026, is the first oral PCSK9 inhibitor. It lowered LDL by 56% to 59% versus placebo in phase 3 trials. Its outcomes trial has not reported yet.
▸How much less does bempedoic acid lower LDL than Repatha?
About 20%, versus about 60% for Repatha. Different trials enrolled different people, so treat both as ballpark figures.
▸Is there a Repatha alternative with fewer injections?
Yes. Leqvio is given once, again at 3 months, then every 6 months. Lipfendra is a daily pill with no injections.
▸What can I take if I can't take statins or Repatha?
Options include ezetimibe, bempedoic acid, Leqvio, Lipfendra and Lerochol. The right choice depends on how far your LDL is from goal and what you have already tried.
