Lipfendra (Enlicitide): The Oral PCSK9 Pill

Lipfendra (enlicitide) is the first oral PCSK9 inhibitor, a once-daily pill from Merck that the FDA approved in July 2026. It lowered LDL cholesterol by about 56 to 59% versus placebo.

What is Lipfendra?

Lipfendra is a once-daily 20 mg pill that lowers LDL cholesterol by about 56 to 59%. The FDA approved it in July 2026, alongside diet and exercise, for adults with high cholesterol, including heterozygous familial hypercholesterolemia.

Every other PCSK9 option needs an injection: Repatha, Praluent, Leqvio and Lerochol.

How well does Lipfendra lower LDL?

About as well as the injectable PCSK9 inhibitors. At 24 weeks versus placebo, LDL fell 56% in CORALreef Lipids and 59% in CORALreef HeFH (people with familial hypercholesterolemia).

In CORALreef Lipids, which enrolled 2,909 people, Lipfendra also lowered ApoB by about 53% and Lp(a) by about 28% versus placebo. 70% of people on it reached an LDL-C under 70 mg/dL along with at least a 50% drop.

How do you take Lipfendra?

Take one tablet each morning on an empty stomach with water, black coffee or plain tea. Swallow it whole, then wait at least 30 minutes before eating or drinking anything else.

The body absorbs very little of the drug even under ideal conditions, so timing counts. In studies, taking it 30 minutes after a meal cut the amount reaching the bloodstream by about half.

What are the side effects of Lipfendra?

Side effects ran close to placebo in the trials. In CORALreef Lipids, 64% of people on Lipfendra and 62% on placebo reported any adverse event, and 3% versus 4% stopped because of one.

In people with familial hypercholesterolemia, diarrhea (7% vs 2%) and dizziness (9% vs 4%) came up more often than on placebo, per the prescribing information.

How much does Lipfendra cost?

Merck set the list price at $10.50 a day, about $315 for a 30-day supply, at its July 2026 approval. That price comes before insurance or discounts. Your cost depends on your plan's formulary and step-therapy rules, or on the pharmacy's cash price if you pay directly.

Does Lipfendra prevent heart attacks?

Researchers do not know yet. Approval rested on LDL lowering, and the outcomes trial, CORALreef Outcomes, is still running.

Statins and the antibody PCSK9 inhibitors (Repatha and Praluent) have shown fewer cardiovascular events in their outcomes trials. Any claim that Lipfendra already prevents heart attacks runs ahead of the evidence.

Common questions

▸Is there a PCSK9 inhibitor pill?

Yes. Lipfendra (enlicitide) from Merck, approved in July 2026, is the first oral PCSK9 inhibitor. You take one 20 mg tablet once a day on an empty stomach.

▸Is Lipfendra better than a statin?

They work differently, and clinicians typically use them together. Lipfendra usually gets added when a statin, with or without ezetimibe, has not brought LDL-C to target, or used in people who cannot tolerate a statin.

▸What is Lipfendra used for?

It is FDA-approved alongside diet and exercise to lower LDL-C in adults with high cholesterol, including heterozygous familial hypercholesterolemia.

▸How much does Lipfendra cost?

The list price at July 2026 approval was $10.50 a day, about $315 a month, before insurance or discounts.
